PROCUREMENT OF NON PNS BLUD EMPLOYEES

SLEMAN DISTRICT HEALTH OFFICE, 2015

The Sleman District Health Office will organize the Procurement of Non-Civil Servants Regional Public Service Agency (BLUD) employees according to the Sleman Regent's Recommendation Number 814/0632/BKD dated 23 March 2015, with the following provisions:

B.   General requirements

a. Indonesian citizens;

b. Have faith in God Almighty;

c. able-bodied;

d. Fulfilling all the specified requirements;

e. Preferably have KTP Sleman Regency;

C.   Special Requirements

  1. 1. General practitioner

a. Have a Registration Certificate (STR) that is still valid at least until 31 December 2015 or a certificate of STR extension from the Indonesian Doctors Association Professional Organization (IDI);

b. Maximum age of 35 years on January 1, 2015;

c. Graduates of the Medical Study Program accredited at least B from the National Accreditation Board for Higher Education;

d. A minimum GPA of 2.75 is preferred;

e. Have work experience in health care facilities;

f. Preferably have a GELS / ACLS / ATLS certificate.

2. Dentist

a. Have a Registration Certificate (STR) that is still valid at least until 31 December 2015 or a certificate of STR extension from the Indonesian Dentist Association Professional Organization (PDGI);

b. Maximum age of 35 years on January 1, 2015;

c. Graduates of the Dentistry Study Program who are accredited at least B from the National Accreditation Board for Higher Education;

d. Minimum GPA of 2.75 is preferred;

e. Have work experience in a health care facility.

3. Psychologist.

a. Have a Psychologist Practice License (SIPP) that is still valid at least until 31 December 2015 or a certificate of SIPP extension from the regional Indonesian Psychological Association (HIMPSI);

b. Maximum age of 37 years on January 1, 2015;

c. Graduates of Psychologist Professional Education accredited at least B from the National Accreditation Board for Higher Education;

d. Minimum GPA of 3.00;

e. Have work experience in health care facilities;

4. Midwife

a. Have a Midwife License (SIB) or Registration Certificate (STR) that is still valid at least until 31 December 2015 or a Certificate of STR Extension from the Provincial Health Workers Council (MTKP);

b. Maximum age of 30 years on January 1, 2015;

c. D.III Midwifery graduates from Educational Institutions accredited at least B from the National Accreditation Board for Higher Education;

d. Minimum GPA of 3.00;

e. Have work experience in health care facilities;

f. Have at least one PONED/IUD/IMPLAN/APN certificate.

5. Nurse

a. Have a valid Nurse License (SIP) or Registration Certificate (STR) until December 31, 2015 or a Certificate of STR Extension from the Provincial Health Workers Council (MTKP)

b. Maximum age of 30 years on January 1, 2015;

c. Preferably graduates of D.III Nursing who are accredited B from the National Accreditation Board for Higher Education;

d. Minimum GPA of 3.00;

e. Have work experience in health care facilities;

f. Have PPGD certificate;

g. Preferably male

6. Sanitarians

a. Have a Registration Certificate (STR) that is still valid at least until 31 December 2015 or a certificate for submitting STR from the Provincial Health Workers Council (MTKP);

b. Maximum age of 30 years on January 1, 2015;

c. D.III Environmental Health graduates who are accredited at least B from the National Accreditation Board for Higher Education;

d. Minimum GPA of 2.75;

e. Have work experience in a health care facility, preferably at a BLUD health center;

7. Laboratory Administration

a. Have a Registration Certificate (STR) that is still valid at least until 31 December 2015 or a Certificate of Application for STR from the Council of Provincial Health Workers (MTKP);

b. Maximum age of 30 years on January 1, 2015;

c. D.III Health Analyst graduate with a minimum accreditation of B from the National Accreditation Board for Higher Education;

  1. d. Minimum GPA of 3.00;

     e. Preferably have work experience in health care facilities.

8. Physiotherapist

a. Have a Registration Certificate (STR) that is still valid at least until 31 December 2015 or a Certificate of STR Submission from the Provincial Health Workers Council (MTKP);

b. Maximum age of 40 years on January 1, 2015;

c. Preferably graduates of D.III Physiotherapy accredited A from the National Accreditation Board for Higher Education;

d. GPA 3.00 preferred;

e. Have work experience in a health care facility, preferably at a BLUD health center;

9. IT and Asset Manager

a. Maximum age of 35 years on January 1, 2015;

b. D.III graduates from all majors accredited at least B from the National Accreditation Board for Higher Education;

c. Minimum GPA of 2.75;

d. Preferably have work experience in a BLUD health service facility;

e. Have general expertise:

1). Mastering all Microsoft Office programs;

         2) Able to type on computer fast

f. Have special skills:

1). Ability in the field of computer network management;

2). Ability in the field of managing the health information system data base.

10. General Administrationa.

a. Maximum age of 37 years on January 1, 2015;

b. Minimum high school education;

c. For university graduates, the preferred GPA is 3,00;

d. For high school graduates/equivalent, the average certificate score is preferred 7,00;

e. Have work experience in the BLUD Work Unit;

A.                   Administration Requirements

  1. Application letter written on his own hand on 6000 stamped paper addressed to the Regent of Sleman UP. Head of Sleman District Health Office;

  3. Legal photocopy of last education diploma and grade transcript;
  4. Legal Photocopy of Certificate of Police Record (SKCK) issued by the local Police;
  5. 3 pieces of 4 x 2 cm color passport photos;
  6. Â Â Â Â Â Â Â Â Curriculum Vitae;
  7. Â Â Health Certificate issued by a government doctor;
  8. Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â A valid photocopy of Registration Certificate (STR)/ Psychologist Practice License (SIPP)/ Nurse License (SIP)/ Midwife Permit (SIB) which is still valid at least until December 31, 2015 or Certificate Extension/submission of STR from the Provincial Health Workers' Council (MTKP), according to the specific requirements of each profession;
  9. Â Â Â Â Â Â Â Â Â Â Â Â Â Â Â Photocopy of Study Program Accreditation Certificate issued by the National Accreditation Board for Higher Education (BAN PT) for those whose certificates do not include accreditation information from BAN PT;
  10. Certificate of Work Experience signed by an authorized official for those who have work experience;
  11. Photocopy of the certificate in accordance with the requirements of each position.

Acceptance of application documents is carried out on Wednesday to Thursday, 25 to 26 March 2015, 08.00 WIB to 14.00 WIB in Hall C of the Sleman District Health Office.

B.      Selection Stage

 

  1. Announcement of administrative selection results on Monday, March 30, 2015 at 14.00 WIB via the Sleman District Health Office website (www.dinkes. slemankab.go.id).
  2. The interview exam was conducted on Tuesday, March 31 2015, at 08.00 WIB – finished (according to schedule) at the Sleman District Health Office
  3. The Practice Exam (specifically for IT and Asset Management Position applicants) will be held in conjunction with the interview exam on Tuesday, March 31 2015, at 08.00 WIB – finished at the Sleman District Health Office.

C.                   Announcement of Selection Results

Announcement of the results of the 2015 procurement of non-PNS BLUD employees at the Sleman District Health Office was held on Thursday, April 2, 2015, at 12.00 WIB via the Sleman District Health Office website (www.dinkes.slemankab.go.id).

D.                  Etc

  1. After being declared passed, prospective Non PNS BLUD Employees will file their filings on Monday, April 6 2015 at 08.00 WIB to 12.00 WIB by submitting the following complete documents:
  2. Certificate from the agency responsible for manpower (Job Seeker Card);
  3. Stamped Statement Letter;

- Willing to enter into a work agreement with the Work Unit of the Regional Public Service Agency (UPT Puskesmas, UPT JPKM, UPT Health Laboratory)

- Willing to be placed in all areas of Sleman Regency;

- Not demanding to be appointed as CPNS and permanent employees.

Applicants who pass to the final selection will be disqualified if:

a. Unable to complete the required files by Monday, April 6 2015 at 12.00 WIB;

b. Resign;

c. Unable to show original diploma documents, transcripts, Registration Certificate (STR)/ Psychologist Practice License (SIPP)/ Nurse License (SIP)/ Midwife License (SIB)/ Certificate of Extension/Submission (according to special requirements for each position) during filing.

3. Applicants who are declared to have passed the final selection, then resign are subject to sanctions not being able to take part in the selection of employee procurement at the next opportunity.

4.  Applicants who have passed the final selection and are declared disqualified will be replaced by the next applicant based on ranking order value according to position formation.

5.  Announcement of selection results is final and cannot be contested.
